Make sure your doctor or midwife completes the Proof of Birth declaration on the back page of the Newborn Child Declaration form.
Medicare will add your newborn baby to your Medicare Card once they are enrolled in Medicare.
You should enrol them as soon as possible.
According to Medicare, a newborn baby is up to 52 weeks of age or their first birthday.
After the birth, your hospital or midwife will give you a Parent Pack including a Newborn Child Declaration form.
There are 3 steps to add a newborn to your Medicare Card:
Step 1:

Step 2:
If you have a Centrelink Customer Reference Number and Medicare Card, add these numbers to the back page of the Newborn Child Declaration form.

Option 2:
Use your Express Plus Medicare mobile app.
